Your User Name is the first letter of your first name, the first letter of your last name, and the last 6 digits of your social security number.
EXAMPLE: Name is John Smith, Social Security Number is 123-56-7890, User Name would be: js567890

Your Password is the 2-digit month, 2-digit day, and last two digits of the year in which you were born, in numeric form: mmddyy
EXAMPLE: Date of birth is May 3, 1978, Password would be: 050378
